Liability Waiver & Release of Claims

Location: Hollywood Casino, Grantville, PA

1. Acknowledgment of Risk

I understand that participating in Seminar Stack  involves physical activity—including martial arts instruction, conditioning, and combat sports—that may result in serious injury, illness, disability, or death. I knowingly and voluntarily accept all associated risks.

2. Medical Fitness

I confirm I am physically fit and have no health conditions preventing my participation. I accept full responsibility for consulting a physician beforehand and for any medical expenses resulting from injury or illness.

3. Release of Liability

In exchange for being allowed to participate, I release Seminar Stack LLC, its owners, staff, volunteers, instructors, sponsors, venue, and affiliates ("Released Parties") from any and all claims for injury, loss, or damage—even if caused by negligence.

4. Assumption of Responsibility

I agree to act responsibly and follow all safety instructions. I will not engage in reckless or dangerous behavior.

5. Indemnification

I agree to indemnify and hold harmless the Released Parties from any costs or liabilities—including legal fees—arising from my actions or participation.

6. Media Release

I grant Seminar Stack LLC permission to photograph, film, and record me during the event. I allow the use of my image, name, and voice for promotional and commercial purposes without compensation.

7. Severability & Governing Law

If any part of this waiver is found unenforceable, all other parts remain valid. This waiver is governed by the laws of the Commonwealth of Pennsylvania.

8. Acknowledgment & Signature

I have read and understand this waiver. I knowingly give up legal rights, including the right to sue, and sign it voluntarily.
